Northern Tier Center for Health (NOTCH), is a mission driven primary care practice with a focus on quality and continuity of care, they are our areas Federally Qualified Health Center (FQHC).  NOTCH is a not-for-profit corporation governed by a community-based Board of Directors whose mission is to provide high-quality care in Vermont’s Franklin and Grand Isle counties.  NOTCH’s team of compassionate and devoted health care professionals and support staff develop long-term relationships with patients, often serving multi-generational families. Northwestern Counseling & Support Services (NCSS), has been serving individuals and families throughout Franklin and Grand Isle Counties since 1958. While addressing the social influencers of health the Agency serves over 4,000 individuals in our offices, in local schools, in the community, in their homes, and in their places of work.
